The development and implementation of an innovative digital referral system between renal and heart failure services in Salford (and referring centres), to improve patient management and outcomes. 

A Collaborative Working project between Boehringer Ingelheim Limited (BIL) and Northern Care Alliance NHS Foundation Trust to deliver integration and efficiency within the Heart Failure (HF) pathway - where management is limited by renal complications, through the creation of a secure virtual cardio- renal platform and service workflow. 


This project will create a secure platform for secondary to tertiary consultation and referral management for the renal teams at the Northern Care Alliance NHS Foundation Trust, referring centres across 6 localities and its integration with Salford Royal Electronic Hospital records (EHR). The referral service itself integrates with EHR; Advice &Guidance (A&G) service is in a separate platform, once a discussion/case is completed, then the records move onto the Electronic Patient Record (EPR). 

The key objectives of this project are;

  • To develop a secure and scalable virtual Muti Disciplinary Team platform to allow the timely optimisation of care for patients with HF and Chronic Kidney Disease (CKD), where advice and guidance in a speedier way can improve patients’ symptoms and outcomes. 
  • To develop a workflow for a virtual cardio-renal service that can be shared wider across systems/regions, and nationally
  • To continue forming collaborative and creative engagement with diverse stakeholders from across an integrated healthcare system to transform care pathways.

The proposed virtual cardio-renal service in this collaboration will build on Salford’s integrated CKD service (virtual primary care monitoring dashboard and multimorbidity clinics) aimed to improve care for patients with CKD and solve the remaining challenges in optimising the care of cardio-renal patients.

The main aim is to develop a new pathway to provide timely access to specialist input for patients with heart failure where management is limited by renal complications. By supporting the development of the virtual cardiorenal service.

It is hoped the evolution of the Patient Pass digital platform will support communication with specialist teams for timely interventions to optimise pharmacotherapy and reduce referrals or acute admissions.

This project aims to mobilise resources for a period of 24 months from October 2023 until end of November 2025.

Boehringer Ingelheim Limited is pleased to collaborate with Northern Care Alliance NHS Foundation Trust to design and deliver this Collaborative Working Project.
