A partnership between Toronto’s University Health Network (UHN) and Boehringer Ingelheim Canada is helping to expand a successful UHN care model that empowers better management of heart failure at home, enhancing health outcomes in the process.
The model, which levers a digital solution called Medly, empowers people living with heart failure to take a more active role in managing their condition by submitting daily readings and receiving instant and actionable feedback. Through Medly, people living with heart failure have access to a nurse, who actively engages with them while also collaborating with their cardiologist and family physician. The approach particularly benefits those who can’t easily visit a cardiologist or family physician.
A standard of care at UHN since 2016, Medly results demonstrated a 50% reduction in the number of heart failure-related hospitalizations and a 24% reduction in all-cause hospitalizations in peer reviewed journals.
Thanks to the program’s success, Boehringer Ingelheim Canada and UHN are working to expand Medly to additional communities with unmet needs, to increase access to specialty services for people living with heart failure. The collaborative nature of the partnership supports strategies tailored to meet local healthcare needs and deliver solutions to advance clinical priorities, while improving quality of life for people living with complex conditions.
The ultimate goal? Sustainable improvements to the healthcare system.
